Injury cause   Tried to raise the pedestal on a TV studio camera by yanking up on pedestal wheel, unaware that pedestal mechanism was locked.
Suspected pre-rupture symptoms -felt a pop in my bicep.  Thought it was just a sprain at first.

Positive aspects of surgical and post-surgical treatment (limited to factors within surgeon’s control)  Doctor very supportive and sensitive to my needs and concerns.
Negative aspects of surgical and post-surgical treatment (limited to factors within surgeon’s control)  Half-cast I got 1st week post-op was too tight on wrist, caused a blister.
Number of incisions (one or two)    One.  Doc told me pre-op that his method is to go in one incision, and decide once he's in whether a second incision is necessary.  In my case I guess it was not.
Attachment type (transosseous sutures, suture anchor(s), endobutton, etc.)  Endobutton.
Nerve block (yes or no)  Not sure
Post-op treatment (time in cast, ROM brace, etc.)  1 week in cast, then 2-weeks ROM brace w 45 deg extension, 120deg flexion. -Then doc set ROM brace to full ROM.  Still use it at 4 weeks post-op, but not mandatory.  Mainly use it as reminder not to actively use my bicep.

Complications: 
*at 4 wks post-op, still have numb skin around forearm and incision site, and index finger feels stiff.
Time to satisfactory resolution of complication(s)  still not resolved
*10 days post op, had constipation, bloody stool, and hemorroids related to percoset pain med
